Do I Have Any Symptoms of Attention Deficit Hyperactivity Disorder (ADHD)?

This survey is designed to provide a quick assessment of whether you might have signs or symptoms of ADHD. However, no test is 100% accurate. No matter what your score is, you should seek help from a health professional if you have any concerns about yourself or your loved ones.

The questionnaire used here is the Adult ADHD Self-Report (ASRS) 6-question screener scale. It is mainly used in adults, however some research suggests it may be valid for screening adolescents as well.
