How Am I Doing With Anxiety?

This survey is designed to provide a quick assessment of whether you might have signs and symptoms of anxiety. However, no test is 100% accurate. No matter what your score is, you should seek help from a health professional if you have any concerns about yourself or your loved ones.

The questionnaire used here is the GAD-7 (Generalized Anxiety Disorder-7 item). Although this questionnaire is used mostly to detect one general anxiety condition, it can also help detect panic disorder, social anxiety disorder, and post-traumatic stress disorder.

If your total is 5 or above, you may be experiencing mild anxiety, and if it is 10 or above, you may be experiencing moderate to severe anxiety. The GAD-7 is scored by assigning scores of 0, 1, 2, and 3, to the response categories of "not at all," "several days," "more than half the days," and "nearly every day," respectively. GAD-7 total score for the seven items ranges from 0 to 21. Scores of 5, 10, and 15 represent cutpoints for mild, moderate, and severe anxiety, respectively.
